le cadre de la construction de son \u00c9tablissement Avec M\u00e9dicalisation (EAM) hors les murs, afin d\u2019accompagner des adultes en situation de suivantes :<\/strong><\/p><ul><li>\u00c9valuer les capacit\u00e9s et les besoins : r\u00e9aliser des bilans psychomoteurs pour identifier les besoins sp\u00e9cifiques des personnes accompagn\u00e9es ;<\/li><li>Interventions th\u00e9rapeutiques : concevoir et animer des s\u00e9ances de r\u00e9\u00e9ducation psychomotrices, individuelles et collectives, pour am\u00e9liorer la coordination, l\u2019\u00e9quilibre et les fonctions motrices des personnes accompagn\u00e9es et pour am\u00e9liorer leur confort global ;<\/li><li>Adaptation de l\u2019environnement : proposer des am\u00e9nagements et des adaptations pour favoriser l\u2019autonomie et le confort des personnes accompagn\u00e9es ;<\/li><li>Accompagnement : accompagner\/ guider les personnes lors d\u2019activit\u00e9s adapt\u00e9es (ex : physiques ou sportives, culturelles, cr\u00e9atives, etc) ;<\/li><li>R\u00e9aliser les \u00e9crits professionnels : bilans, transmissions, projet d\u2019activit\u00e9 et personnalis\u00e9 ;<\/li><li>Collaboration pluridisciplinaire : travailler en collaboration avec les \u00e9quipes soignantes et \u00e9ducatives pour assurer une prise en charge globale et coh\u00e9rente.\u00a0<\/li><li>Participation aux r\u00e9unions pluridisciplinaires et au projet personnalis\u00e9 ;<\/li><li>Suivi et \u00e9valuation : assurer le suivi et l\u2019\u00e9valuation des progr\u00e8s des personnes accompagn\u00e9es et adapter les interventions en cons\u00e9quence ;<\/li><li>Formation et sensibilisation : participer \u00e0 la formation continue du personnel et sensibiliser aux bonnes pratiques psychomotrices ;<\/li><li>Accompagnement des familles : informer et conseiller les familles sur les besoins et les progr\u00e8s des r\u00e9sidents.<\/li><\/ul><p><strong>Profil et comp\u00e9tences requises<\/strong><\/p><p><em>Vous \u00eates titulaire du Dipl\u00f4me d\u2019\u00c9tat de Psychomotricien et vous avez une
